I personally don't think The Sultan's Harem has enough in it to justify $40. A lot of the dialogue is recycled when it doesn't make any sense which needs to be fixed. I'm still annoyed that he added new stages to Lela with no special events.

One massive downside of it though is some scripted events can mess up when the NPCs are fast forwarded, but that's something avoidable and depends on the insight of the dev as well, there is a "right" and a "wrong" way to execute events, and a lot of creator's I've seen don't do it properly.
