I've got to confess, feeder Misato/feedee Shinji isn't a pairing I've given much thought to. Misato makes a great pervert, and of course it's canonical to the anime that she can wrap Shinji around her finger when she wants to. There's definitely at least a point in the progression of the show where, if she'd wanted to fatten him up, he'd probably have gone along with it.

I think it would be interesting to do one contrasting Rei and her treatment of Asuka with Kaworu's treatment of Shinji, and the differences. Kaworu despite the more sickly sweet vibes is very much a lover and the whole thing has domination and humiliation vibes, meanwhile Rei is more of a motherly mollycoddler spoiling Asuka into forgetting all her pain by gradually regressing her into an infantile state. I'd love to complete that contrast with a third party(maybe lardy Misato) pondering health upon her own chest pains, with Kaworu basically saying if fate wills it and hinting there have been other Shinji's in other timelines before this he's fed to death, while Rei gets visibly uncomfortable with that whole thing and vows to 'take good care of Asuka' with the implication she'll use her Lilith powers to keep Asuka somewhat well. That and I get the sense if Asuka EVENTUALLY decided to try growing up again and slimming down enough to be mobile Rei would let her (albeit still being the mommy figure every step of the way to ensure Asuka grows up happy this time and probably keeping her at least somewhat fat and spoiled), while Kaworu wouldn't let Shinji go even if he wanted to go

Basically, Shinji only has complications if Kaworu wants him to have them. If any of them in the Rentry stories are going to keel over first, it'll be Misato. Not only is she the biggest of them, she's got no caretaker with supernatural Angel powers. I assume it's nothing more than her own hedonistic spirit that's keeping her going at this point. I can maybe see Kaworu allowing Shinji to die, but only as a final act of domination/humilation. He'd definitely want Shinji's final words to be "I love you, Kaworu-san" as his eyes roll back into his head and his heart stops beating. Meanwhile, you're correct that Rei would allow Asuka to slim down if she really wanted to. But Rei would probably try to arrange Asuka's life so that the thought never crosses her mind, either, since she loves babying Asuka so much.

I can’t remember who said it, but someone once pointed out that Shinji thinks he wants parental affection, but actually wants carnal affection, while Asuka claims to want carnal affection, but actually wants parental/maternal affection. And it really shows in these stories. Shinji’s stuff inevitably takes an incredibly sexual tone while Asuka if given the chance reverts back to the crying girl deep inside who just wants to be taken care of and loved

Rei is one of our protagonists; she's part of the core cast. Even when we learn about her otherworldly origins she never stops being somebody we're encouraged to like, and sympathize with. Whereas Kaworu is one of the straight-up Angels. However kind and friendly a face he may put on, his ultimate goal is to merge with Adam, trigger Third Impact, and end the world. Asuka's paired with somebody the show treats as a hero and protagonist, Shinji's paired with somebody the show treats as a villain and antagonist, it's entirely natural that their pathways to fattening and caretaking are fundamentally different. Frankly I think Kaworu gets glazed a lot by more modern viewers when he doesn't deserve it. As Shinji himself realizes in the canon turn of the episode, Kaworu DOES deceive him, and doesn't necessarily have his best interests at heart, even after saying "I love you." Rentry story Shinji never gets to that realization, and basically it destroys him. Whereas you really get the sense that Rei's not out to hurt anybody.

I enjoy writing that too, that's a bit of what I was talking about in my post. It's important to keep in mind that Shinji is genuinely happy like this, happier than he ever was piloting Eva or being in Tokyo-3. As I've said in the past, Kaworu provides him with genuine affection and asks for no emotional labor on his part. Shinji laps this up like a cat lapping up milk. And so knowing that Kaworu wants him fat, he'll get fatter. Knowing that Kaworu likes him feminine, he'll get girlier. Knowing that Kaworu finds it charming that he's lost bowel control, he'll wallow in his own feces. To Shinji it's all worth it to feel Kaworu's love. Not to mention I think he enjoys at least some of this in its own right, thanks to Kaworu's careful grooming of him. I'm pretty sure Shinji has the "mind" of a fat person at this point, actively being a greedy glutton who lusts after food. He's got a pretty serious sweet tooth, as you can tell.
