Its usually a sign of congestive heart failure. Basically your heart struggles to properly pump blood throughout the body and waste fluids properly and they collect in the calves and ankles due to gravity.
So it is a very bad thing both health and aesthetically speaking. A little bit of cardio or light exercise on semi-weekly basis is usually enough to prevent it from developing or progressing since it usually helps with keeping the heart strong and having it work harder in short bursts helps tremendously with circulation in everyone regardless of physique. Also being completely sedimentary or outright bedbound is basically going to guarantee this will happen to you at some point.
Not factoring in daily exercise, how bad it gets depends on genetics, fat distribution type, stress levels, heart size, heart health, blood pressure, diabetes management, height, diet, and even how you sleep. If these factors are too extreme then exercise alone isn't going to do much.
The human body also just starts to run into a hard limit as it gets bigger. because the heart gets less efficient at transporting blood and removing waste as you get bigger. Because their is physically larger area for it to cover, using thinner blood vessels, and while the heart can grow, it can only get so big before it physically can't grow anymore. For most women, this weight range where you are basically guaranteed to get it seems to range from roughly 450lb to 600lb depending on genetic, diet, and lifestyle factors. But there is really nothing that can truly stop it other than losing weight once past that weight threshold.
Granted I am not a doctor, I just read a lot about this because I don't want BBWs getting heart disease because losing beautiful people to preventable death or grotesque disfigurement is just sad.
